This purchase is an auto-renewing contract. By purchasing this membership you understand: You are contracting the services of Erica Roselius, and/or Driven Fitness to provide personal training/small group training beginning the date of purchase. All Sessions are by reservation only, applicable cancellation policy applies. Training EFT is on a month-to-month basis. Driven Fitness reserves the right to raise session prices at any time. Your membership auto-renews every four weeks (after the initial term expires if applicable) and you have the entire month to utilize all of your training sessions (if applicable); and you acknowledge that it is your responsibility to schedule your group sessions accordingly with your schedule. It is your responsibility to utilize your credits each billing cycle. You further acknowledge that at the end of each billing cycle, unused credits will rollover to the next month to be used as extras. Unused credits will not replace the package you committed to purchasing each billing cycle per this contract. Unused credits will remain active and can be used provided you are under an active membership agreement. YOU UNDERSTAND THAT YOU ARE IN FULL CONTOL OF YOU PAYMENT, AND IF AT ANYTIME YOU DECIDE TO MAKE ANY CHANGES, YOU WILL WRITE TO OR VISIT DRIVEN FITNESS. CHANGES MUST BE MADE AT LEAST 30 DAYS PRIOR TO THE START OF THE BILLING CYCLE. CANCELLATIONS MUST BE MADE AT LEAST 30 DAYS PRIOR TO THE START OF THE BILLING CYCLE. DRIVEN FITNESS DOES NOT MAKE CHANGES ON THE CURRENT MONTH OR GIVE REFUNDS FOR THE CURRENT MONTH. EFT AUTHORIZATION: You authorize Erica Roselius dba Driven Fitness or its assigns to make periodic charges or withdrawals (“EFT Authorization”) from the account used to pay the initial payment or the account selected or a replacement account designated by Client and accepted by Erica Roselius dba Driven Fitness. You understand that you are entitled to notice of all varying charges and withdrawals under the EFT Authorization. You may change the account designated herein upon fourteen (14) days written notice to, and approved by, Erica Roselius dba Driven Fitness. Client may timely notify the financial institution in control of Client’s account to terminate this EFT Authorization, but such notification will constitute a default and may cause all sums under this Agreement to be due and payable immediately together with all costs of collection to extent permitted by law. You may change or cancel your auto-renewing contract at any time with 30 days prior written notice (or after conclusion of initial term if applicable). You acknowledge that your rate may change upon making these changes. 24 hour cancelation policy: We have a 24-hour cancelation policy for all in-studio classes and all semi-private or private sessions (in-studio and zoom). You must cancel with at least 24 hours notice or you will be charged for that class. If you are on an unlimited membership, you will be charged a $5 late cancelation fee. You acknowledge that should you cancel your agreement any unused credits are forfeited. Driven Fitness does not issue refunds for unused credits. Vacation freezes can be applied for vacations over 3 weeks once per year with a written email notice. If you choose to terminate during your freeze, you will be charged your final month’s payment to close out your contract as per our 30-day cancellation policy. Hold and/or Leave of Absence Policy. You may place a hold on your membership at any time with 30 days written notice. Driven Fitness requires a stop and re-start date for all holds. Hold/Leave of absence is allowed with a written letter from a doctor and with a written email.
